Fire Report Request Form

If you are a resident, business owner, insurance adjuster or fire investigator and are in need of a fire incident report you may submit a request by completing the information below. There is a $25.00 fee for obtaining a copy of a report. This is to be paid by check to:

"Phoenixville Fire Department" at 150 Paradise Street, Phoenixville, PA 19460.
